Day Services
Day Habilitation Services
Excellent Minds Inc. aimed at developing activities and /or skills acquisition to support or further integrate community opportunities outside of a Person’s home, to foster independence, autonomy, or career exploration, and encourage development of a life within the community.
Individualized Day Support (IDS)
Excellent Minds Inc. provides crucial habilitation support in the community to ensure that a person’s community integration is increased and the skills necessary for independence and community involvement outside the home are developed and maintained in ways that enhance community integration outcomes, based on the individualized preferences and goals.
Residential Services
Companion Services
Companion services provide non-medical assistance and supervision to support a Person’s goals, desires, and needs as identified in the Person’s Individual Support Plan (ISP), and reflected in his/or her Person-Center Thinking and Discovery tools. It also includes flexible list of proposed leisure and recreational activities at home and in the community, based upon the Person’s interests.
Host Home Services
Excellent Minds Inc. has reliable and credible family-type setting accommodations that will support Person’s in this program to achieve their goals, participate in community life and activities, maintain their health, and retain or improve skills that are important to them, which may include ADLs, such as grooming, bed making, and household chores, money management, travel, recreation, cooking, shopping, use of community resources, community safety, and social and other adaptive skills they identify that are needed to live in the community.
In-Home Support Services
Excellent Minds Inc. supports Persons living independently in their own home or living in/with their family members or a friend’s home. In-Home Support Services are blended services that provide a combination of hands-on personal care, habilitation support, skill development, assistance with ADLs and other support services in their home.
Respite Care Services
Are you a family member providing care and need a short time off? No worries, Excellent Minds Inc. Respite Care will ensure that Persons will continue to receive services and have access to community activities as described in their ISP. Respite is provided on an hourly, and/or daily basis.
Supported Living Without Transportation Services
Excellent Minds Inc. has several credible DDS-certified housing options to support a person under this program. Supported living is a blended service that covers habilitation, In-Home Support, Nursing, and other residential support services.
Clinical Services
Behavioral Support Services
Excellent Minds Inc. offers non-professional Behavioral Support Services through its Direct Support Professional (DSP). This service is aimed to assist in improving the Person’s independence and inclusion in their community. Excellent Minds Inc. provides effective Behavioral Support (BSP) training to its DSPs based on the Person’s Behavioral Support Plan (BSP Plan). This guarantees safety and community inclusion for the Person supported.
Skilled Nursing Services
Excellent Minds Inc. provides medical and educational services through its RNs that address health care needs related to prevention and primary healthcare activities. These activities include health assessment and treatment, health-related training, and education.
Professional Support Services
IDD Parenting Services
Excellent Minds Inc., in this service, will support ID/IDD Persons who are or will be parents in developing appropriate parenting skills. Excellent Minds Inc. will provide training that is individualized and focused on the health and welfare and developmental needs of their child, as well as building necessary parenting skills. Close coordination will be assured and maintained with informal and other formal support (Excellent Minds Inc. DSPs).
